Here is a idea, not sure how to put it though...
Hope this is understandable.
D3 could start off, almost like an expansion, by taking your existing character(s) and continuing where it left off. Tyriel portals the your hero off right before shattering the worldstone. Where does the hero go?
Where ever it was suppost to be, the shattering of the worldstone disrupted that, and sent the hero to the demention that the Prime evils come from.
This world is a complete opposite from your own. Trees grow underground, but the roots are exposed to the sky. water flows upstream ect. Even the image of you is monsterous. (Depending on what class of character will determine what you look like. For example a Minitor for a Barb)
Appearing outside of a town, everyone else looks normal, (human) but you come off as some monster, that no one can understand. All are affaid and run away and/or attack you out of fear.
(basically crossing over into this world you experience the minion side of things, and face what the other side must have felt)
As you go on, you realize their is a artifact collector that has found shards of a stone he calls "stone of the world". These fragments are put together, and create a ever increasingly wave that will engulf the world.
This wave will take the world you are in and combine it with the world you are from forever. The main goal is to stop this from happening, but the lesser of the prime evils have traveled here to take the stone back to their world to use it as a doorway back to the Diablo2 reality.
You find a old man called the wanderer, he somehow can understand you, and, you can speak to him. He begins to tell you that, he also has fought against Diablo, but in order to contain him, he had to merg his body with the soul of Diablo threw the soulstone. Or so he thought. Upon doing so he arrived here. You begin to tell your history and what further happened to you as it trails off.
It is explained to you that a witch in the hills outside of the city can cast a spell on you, which will make you look normal, and talk and listen to everyone else. You venture there fighting against heros from D1 & 2 builds. The only difference is very suttle skill differences for them.
(example would be a bliz sorc and a wind druid can together use their bliz and hurricane skill to create a new skill called "Hail" crating a hurricane with balls of ice that fall from the sky, while protecting them both ect.)
You on the other hand, will have the same level of exp as you did in D2, but your skills will be altered to suit your new form.
(example if you were a wirlwind barb you will now be a minitor using the skill wild swing. Same thing only you swing wildly at every attacker maybe hitting them depending on %)
After reaching the witch, she tells you that everyone here, is in one way or another a decendent of someone who has fought against the prime evils. It turns out that, the whole thing is one big giant circle in time.
The witch also tells you if you are changed to a normal person here, you will be without your skills, and that you must start over. Here you have a choice of become a new character of good, with new skills ect, or bad lvl 1 with whatever class of character you are now, but no memory of things that have happened to you up till now.
From here their is two different story lines to take. Good - find your way back to your world, or bad - try to use the stone of the world to get into the Diablo universe.
Good side: (what actually happened between D1 & 2)
Your goal is to get back to the Diablo universe and stop the chain of events that have happened. You enter tristrum just as the demons attack it in Diablo 1, unfortunitlly you are unable to stop Diablo before he destroys the town. As the story goes on you must you find out you are the orphan child of the hero from D1, but you must kill your father to keep him from becoming Diablo again in D2, then you must kill off Teriel before he can destroy the worldstone and keep the chain of events going.
Badside: (who is the worst evil you, or the Prime Evils)
Use the stone of the world to get to the prime world and try to overthrow the D1 & 2 Universe. Also you will be at every major battle from both series, and destroy the lesser and prime evils for good. The goal is to kill off all to gain total control of the universe. What transpires is that you are the only one that can actually destroy any of them, if a "hero" does so it only sends them back to their world. You find out threw a quest that "Only evil can destroy evil."
In either case, good or bad, you find that nothing can stop what has been done. This loop will continue in time forever.
Side note:
Before every battle with a prime evil their will be a cut seen where it shows that prime battling against some heros. They leave thinking that the evil is vanqueshed. Unknown to them their is a skill that the evils have called spirit transmission. That when near death, they can transfer their soul into another and have that others soul put into their actual body. In the end the body is gone, but the soul remains, only to regenerated soon after, and the prime can transmit back to its body. This is where your character comes in to attack.
