just for you people who believe this to be a hoax or a joke, this was originally posted by SaladFork on RPGForums.com, original link found here
Saladfork is a forum officer and a very trusted member on those forums, so there's some backing to this rumor. here's the post, followed by his replies with more hints:
i don't plan on trying anything on closed battle.net until i find out the recipe for sure. i'm off right now to make a open character with tons of keys/body parts/runes/standards/whatever and see if i can find something out. it looks like theres a combination to open another portal, so i'll be trying everything in act5. i thought you guys might find this interesting, even if you think it to be a hoax. if anyone hears any more rumors/facts/has any ideas on this, post them here.
enjoy!

And now, some pictures:
My Stash. You can see some items that you will need in order to obtain these charms.
Divine Intervention Small Charm. I got a really luck roll, =)
Demonic Intervention Small Charm. An average roll, no complaints.
I want to keep this as a specialty, so I'm not going to give away any specific information for obtaining them. However, I will give you guys some hints, and set you on your way.
* You are going to need one or more items called "Standard of Heroes".
* Think in the past to another item you have obtained that was a unique small charm, and was a quest item, and had ZERO magical attributes. I'll give you a hint, it used to belong to Mephisto. Even though it looked like it did NOTHING, and there were no recipes for it, if you used it at the right place in the right way, you got results.
* You are going to need to battle some Corrupted Horadric Mages. They are not easy, and this will probably be the hardest battle you have ever fought. Think a combination of the 3 ubers, and the hell ancients. Once they are killed, you will fight Uber Tyrael. It turns out he was corrupted by the soulstone.
* When killed, a passage will be read to you (sort of like when you kill the ancients). I would highly recommend reading it, as it's very interesting, and makes sense if you've been following the Diablo II story.
* Only one charm will be dropped upon his death, no matter how many players are in the game. You can only carry one of each charm at any one time. I think that the drop is random, though I have gotten more Demonic's than Divine's.
* No more screenshots or proof will be posted. I have already given away too much information. This was supposed to be kept secret so only a select few knew about it, but it is being shared with you guys. If you want to know more, or to have your own, you're going to have to do some things by yourself.
* I do not claim any responsibility for lost Standards when trying to do this, nor do I claim any responsibility for any misinformation in this post. If I mistyped something, I apologize, I did not mean to.

another reply:The Meph Stone is not part of this at all, I just told you to think back to it, as it is very similar. The Meph stone was a unique small charm, that was a quest item, it also had NO magic attributes.
However, if you used it on the hellforge with a hammer, you get a reward, even though it had no magic attributes or anything like that.

Other than the meph soulstone similarities, the biggest hint I can give you is to look at the screenshot of my stash.
Quite a few people have figured it out already, so I know that my hints aren't THAT bad.
