Ladder Characters?

Ok, I don't really understand the significance of a ladder character in diablo? Is the objective to get to the highest level and have the best Monsterkill/death ratio? Even so I thought they have ladder already right now. Explain to me. Does this involve pvp kills/deaths too?

humm..good point i dunno if pvp counts on the ladder or not!
