Help Nezeramontias mod cant install it

i need help i just re installed diablo 2 lod and put patch 1.10 patch then changed the file name in the Nezeramontias thing to d2_patch.mpq or what ever and when i run it it comes up with errors

yeah ok but how can i, i dont think i have installed it properly

Rename the original patch_D2.mpq file to patch_d2.old and the new .mpq file to patch_d2.mpq.

that's what it's all about
